Standalone Weight Loss Follow-Up Visit
For patients not enrolled in the Weight Loss Bundle who require ongoing prescription management.
Required every 6 months for continued GLP-1 prescriptions.

- 45-minute visit to review your weight trends, side effects, labs, and wearable data
- InBody body composition scan
- Prescription adjustments and clinical guidance

What’s Not Included
- Prescription medications: GLP-1 agonist costs are separate and variable based on your choice of agent and dosing. We provide discounted access through Strive Pharmacy. We also provide brand name Wegovy and Mounjaro through local or mail in pharmacies.
- Additional lab work: Beyond the included baseline and 3-month labs in the bundle, additional labs may be ordered and billed to your insurance or paid out-of-pocket.
- Nutritionist consultation: Available upon request; pricing available separately.
Frequently Asked Questions
Q: Can I start with a standalone consult and then join the bundle?
A: Yes. If you begin with a standalone initial consult ($300) and decide to enroll in the Weight Loss Bundle within 30 days, we will credit $150 of your consult fee toward your first month’s membership.
Q: What if I want to cancel my membership?
A: You may cancel after the initial 3-month commitment with 7 days’ notice. Months paid in advance are non-refundable unless canceled within 7 days of enrollment.
Q: What if I need labs more frequently?
A: Additional labs can be ordered anytime and will be billed separately to your insurance (if eligible) or as out-of-pocket cash pay.
Q: Is telehealth available?
A: Yes. Follow-up visits in the Weight Loss Bundle can be conducted in-person at our Avon location or via secure telehealth. Initial consults are available in-person or via telehealth based on your preference.
